Release of Liability

The following is a release of actual or potential claims for injury.
Please read carefully.

I have decided to participate in one or more activities with, through or sponsored by Medalocity. I acknowledge that my participation in these activities is entirely voluntary. I am aware that participation in any or all of these activities is a hazardous activity. I am voluntarily participating in these activities with knowledge of the danger involved and hereby agree to accept any and all risks of injury and death which may result from my participation.

As consideration for being permitted to participate in these activities, I, individually and on behalf of my assignees, heirs, guardians or legal representatives agree that I will not make a claim against, or sue, Medalocity or any of its agents, representatives, assigns, affiliated organizations, suppliers, contractors or consultants including, but not limited to Rodney Butler, Vidal Arroyo, Brian Miller or Avalon Divine, for any injury, damage or death, how so ever caused arising from or relating to my participation in said activities including, but not limited to claims for the active or passive negligence. I hereby release Medalocity as well as its agents, representatives, assigns, affiliated organizations, suppliers, contractors or consultants from any and all claims, demands, injuries, damages, actions or causes of action, that I, or my assignees, heirs, guardians or legal representatives now have or may have in the future arising from or relating to any damage, injury or death resulting from my participation in any or all of said activities, including claims for active or passive negligence.
